When I read the changelog doc, I found:
2014-02-26 Ludovic Courtès <ludo@gnu.org>
* gdb/doc/guile.texi (Basic Guile): Document 'history-append!'.
I think it should be:

* guile.texi (Basic Guile): Document 'history-append!'.
Is that right?

Yes, you are correct. It's immediate to fix it, so I just went ahead
and did it, rather than wait for someone to fix it.

Yes. File names should be relative to the directory of the ChangeLog
file where they appear.
