This updates insert/remove breakpoint so that they use a bfd_byte*
instead of a char*.
Andrew, these bfd_byte * changes are CONTROVERSIAL. Please revert
this commit and don't commit any new ones until we have at least
agreement among the global maintainers about how to proceed.
