You should use 

fde->initial_location = read_pointer (abfd, &start)
    + ANOFFSET (objfile->section_offsets, SECT_OFF_TEXT (objfile));

instead.

Otherwise it's ok.

BTW: I get 2 warnings compiling dwarf2cfi.c on i386.
